Dear Miami Institute Students,
If you want to go to Miami's Recreational Center - a brand new, amazing facility - you MUST (NO EXCEPTIONS) fill out this Informed Consent Waiver form. If you are flying to Miami, and your parents are not coming with you, you have to print this form out and sign it. If you do not do so, we CANNOT let you go to the Rec Center because we do not want to take responsibility for you.

DEPOSIT INFORMATION 2007
Dear Miami Institute Students,
Our records currently show that some of you have verbally accepted our invitation to join us this summer but have not yet sent in your deposit. If you are one of those people - and you probably know who you are, I would like to please ask you to send in your deposit now, as soon as possible.
If there are any extenuating circumstances, please contact me (vinsonaw@muohio.edu).
